As alterações mais pronunciadas são encontradas nos capilares, onde há tumefação e proliferação de células endoteliais.

Em vasos sanguíneos maiores, as túnicas média e adventícia podem apresentar hialinização ou necrose. Encefalite não supurada (manguito perivascular constituído por células mononucleares) é detectada frequentemente.
