Radiographic Consultations

Please submit a copy of the Radiographic Consultation Request (PDF: 246 KB) form with each set of images. The form may be printed as is and filled out by hand or you may type in the information requested and then print the form.

Images will be evaluated by a Radiologist and a report will be faxed and/or mailed to you.

Fees are as follows:

Radiographic examinations submitted from GA, WV, or SC: $40 / case

Radiographic examinations submitted from other areas: $55 / case

Special procedures, CT, MRI examinations: $50-100 / case

Prepurchase examinations: $60 -100 / case
