AUP Forms

Animal Order Request Form [PDF]

The Animal Request Form can be filled in online. The departmental office manager MUST provide accurate accounting information and sign the request form. Submit three (3) copies to the Animal Resources office (Room 206). Blank forms can be printed, but information must be TYPED on the form.

Services Request Form [PDF]

The Services Request Form can be filled in online. This requisition form covers such AR-provided services as animal rentals, transport, identification, vaccination, etc; labor for special animal care or special services provided by the AR animal health tech; use of the cage washers, autoclave, laundry; room rental; and provisions for special ordered feeds and supplements. Blank forms can be printed, but information must be TYPED on the form.

PI-to-PI Animal Transfer Form [PDF]

This transfer form is to be used ONLY when animals are being transferred from one PI-AUP to another PI-AUP. ALL information must be filled in and the form must be signed and dated by the transferring PI, the receiving PI, and the departmental office manager.

Instructions using Acrobat Reader

The PDF is editable on your computer with Acrobat Reader by following the steps below:

Click on the Animal Request Form - [PDF] link
The PDF will either open in Acrobat directly in your browser window, or will download to your computer depending on the settings you have set up in your browser
Once in Acrobat with the Form open, Hit the Tab key to go to the first Field - "P.I." and enter in the P.I.
Hit Tab again, and the cursor will jump to the next field, complete that entry, and so on.
When all entries are complete, Print the file by selecting File > Print.
Submit the printed form to Animal Resources.

Note: The completed from cannot be saved with the entries unless you have Acrobat Professional. Acrobat Reader will not allow the file to be saved; it can, however, be printed.
